The Placement Specialist will meet with you at the DVR office, your home or another convenient location to gather information about your skills, interests and past experiences. It is not a requirement to meet at the ENABLE office in Livingston. The Director will then review the information provided at intake and make a decision about your acceptance for ENABLE services.
You will need the following items for your intake with ENABLE:
- Copies of your social security card and birth certificate.
- Copy of Photo identification card that includes current name and address (DMV is preferred)
- Current Social Security awards letter stating monthly benefits.
- A list of 2 to 3 references. Include their address and phone numbers and their relationship to you. Former supervisors and/or co- workers are ideal.
